USA Hockey Consent to Treat/Medical Form
All players under the age of 18 must have a parent/guardian sign the USA Hockey Consent to Treat / Medical Form. All players 18+ years of age must sign this form themselves. This signed form gives clearance to any on-site medical professionals to administer care if necessary. The form must be physically signed in ink.
